ERROR: Sorry, missing total_occurrences info

Hi there. I have a customer who is receiving this error message when clicking 'submit' at the bottom of the checkout page. I've tested the button myself and she's cleared out her cache and cookies and restarted her computer - same error message. We're just going to send her a PayPal invoice, but I'm hoping we can avoid this in future. Does anyone know what might be triggering this error? Thanks...

This can happen if there is a .htaccess redirect rule in your .htaccess file that is resulting in the request header losing the info that dap needs.
